DISCLOSURE FOR NEW SOCCER SCHOOL COACHES
Applicants must complete the attached SYFA self declaration form - see attached, prior to the Disclosure meeting.
The self declaration form must be fully completed where asked. Do not omit any information as this will delay the process or even result in your disclosure being rejected.
Applicants must also bring:
1. birth certificate,
2. driving licence or passport,
3. utility bill, e.g. gas bill,
4. Address information including each postcode where you have stayed for last 5 years, and
5. National Insurance number - see your payslip
Applicants who do not have a passport or photo drivers licence MUST bring two passport sized photos signed on rear by doctor, policeman, councillor etc.
Finally, make sure you have your hair done (delete as appropriate) for your ID photograph!
